Inspector Notes
The Licensing Inspector initiated and completed a monitoring inspection on 9/26/2022. The inspector was on site from 2:55pm until 4:30pm.
There were thirty-three children under the care of four staff. The ages of the children ranged from five to eleven years old. The children were observed during free play and snack time. Four children?s record and four staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

4Evidence: Child #1, which was enrolled on 08/15/2022 had no documentation of immunizations on file during the inspection on 08/26/2022.
Evidence: Child #1, which was enrolled on 08/15/2022 had no documentation of a physical on file during the inspection on 00/26/2022.
Evidence: The center had no record of an annual health inspection on site during the inspection on 9/26/2022.
Evidence: Children were observed having snack time. The children had just come in from outside and given hand sanitizer to clean hands. When questioned about washing hands the staff member said they normally take them to the bathroom but today they didn?t.
