Inspector Notes

The Licensing Inspector initiated and completed a monitoring inspection on 9/26/2022. The inspector was on site from 2:55pm until 4:30pm.

There were thirty-three children under the care of four staff. The ages of the children ranged from five to eleven years old. The children were observed during free play and snack time. Four children?s record and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Standard 8VAC20-780-130-A
Based on record review, the center failed to obtain a copy of child?s immunizations before first day in attendance.

Evidence: Child #1, which was enrolled on 08/15/2022 had no documentation of immunizations on file during the inspection on 08/26/2022.
Plan of Correction: Parent will bring updated child's form by 10/7/2022.
Standard 8VAC20-780-140-A
Based on record review, the center failed to obtain a copy of the child physical examination before child?s first day or within 30 days after first day of attendance.

Evidence: Child #1, which was enrolled on 08/15/2022 had no documentation of a physical on file during the inspection on 00/26/2022.
Plan of Correction: Parent will bring updated child's form by 10/7/2022

Based on observation the center failed to provide annual approval from the health department.

Evidence: The center had no record of an annual health inspection on site during the inspection on 9/26/2022.
Plan of Correction: Health Department will be contacted about inspection by 9/30/2022.
Standard 8VAC20-780-500-A
Based on observation and interview, the center failed to make sure children?s hands were washed with soap and running water or disposable wipe before and after eating meals or snacks.

Evidence: Children were observed having snack time. The children had just come in from outside and given hand sanitizer to clean hands. When questioned about washing hands the staff member said they normally take them to the bathroom but today they didn?t.
Plan of Correction: Children will be taken to bathroom to wash hands at all times.
