Inspector Notes

The Licensing Inspector initiated and completed an unannounced monitoring inspection on 4/11/2023. The inspector was on site from 1:50pmuntil 2:30pm.

There were 19 children under the care of 2 staff. The ages of the children ranged from five months to five years old. The children were observed during indoor free play. Three children?s record and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the center failed to ensure each staff member has a documented
TB test or screening every two years.

Evidence: Staff #2 hired on 08/13/2009 had no record of a TB screening on record during the
inspection on 4/11/2023.
Plan of Correction: Staff will be sent for a test by 5/5/2023.

Based on observation the center failed to provide annual approval from the health department.

Evidence:
The center had no record of an current annual health inspection on site during the inspection on 4/11/2023. The last inspection was 3/9/2021.
Plan of Correction: Health Department will be contacted
for inspection.
