Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 9/16/2024 and completed on 9/30/2024, as a part of licensure period, the provisional licensure period. The on-site inspection began at 2:00pm and ended at 3:40pm. The inspector reviewed compliance in the areas listed above. There were 28 children present and 2 staff. The inspector reviewed 5 children?s records and 2 staff records electronically on 9/26/2024.
This inspection included document review, tour of the facility, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/7/2024. A POC submitted after this date will not appear on the public website.
Violations
5There was no physical examination in the record of child #4, who had been enrolled for one month.
1.On the playground equipment, there were cracks in the slide and the tunnel of the playset. Children were observed in this area.
2.On the outside of the pavilion, there were multiple tarps, an animal cage and storage cover on the ground. This is a suffocation hazard to children. Children were observed in this area.
On the outdoor playground there was a bottle of chemicals on a shelf. Children were observed in this area playing.
Several children went into their lunchboxes for a snack while on the playground. There was no handwashing, or disposable wipe before or after the meal.
None of the children?s food containers on the playground were dated.
