Inspector Notes

An unannounced monitoring inspection was conducted on 5/25/2023, a subsequent review of records was conducted remotely on 6/5/23. The HR staff was out on emergency leave.
The center had 106 children present with 20 classroom staff at the time of the inspection. The children were observed napping, having snack, peaches and yogurt, playing, having story time and playing on the playground. Lots of interaction between staff and children. The center is full of materials and centers for the children to explore. The classrooms have planted gardens with lots of vegetables and herbs. Based on the review of 4 staff records, two staff are missing some of the required background checks.
Evidence: Staff 1, does not have clearances from Washington D,C where they have lived in the last 5 years.
Staff 2 filled out the sworn disclosure statement incorrectly, indicating that they had not lived in a different state, other then Virginia in the last 5 years, they currently reside in Maryland.
Plan of Correction: We will make sure we request the background checks from Washington D.C.
We will also make sure the staff corrects the sworn disclosure statement.
Standard 8VAC20-780-160-A
Based on the review of 4 staff records, the center failed to get a tuberculosis screening that was within 30 days of employment.
Evidence: Staff 1 was hired on 1/23/23 and the TB test in the file is from 10/21/22.
Plan of Correction: We will make sure that TB screenings are done within the allowable time frame.

Based on the review of the centers emergency drill, the center failed to have monthly drills.
Evidence: The center failed to conduct a drill in December 2022.
Plan of Correction: We will make sure we record and conduct monthly evacuation drills.
