Inspector Notes

The following inspection took place between the hours of 12 pm through 12:30 pm. Today, I met with the staff of the Primrose School at Moorefield Station CDC located in Ashburn, Virginia. This unannounced Non-mandated Monitoring Inspection (NMI) was conducted as a result of an incident self-reported by the center to the Office of Child Care Health and Safety (Fairfax).

Information gathered during the inspection determined 1 non-compliance with applicable standards. This Violation was documented on the Violation Notice issued to the center.

Standard 8VAC20-780-340-F
Based on interview, a child under 10 years of age was not always within actual sight and sound supervision of staff.

Evidence -
1. The Center Director stated (during an interview conducted on the date of inspection) that on 06/14/2023, at approximately 11:25 am, within the preschool pathways 1 room, Child #1 (2 years of age) was left unsupervised for approximately 5 - 10 minutes.
2. This incident was a result of Staff #1 performing an incorrect name-to-face count of the 16 children leaving their classroom to go outside.
3. Child #1 was found by another staff in the classroom sitting and unharmed.
Plan of Correction: Staff #1 is no longer employed with the Child Day Center. All staff will be reminded of the center's supervision policy and the procedures in how to perform a name-to-face count correctly.
