Inspector Notes

An annual monitoring inspection was initiated on 02/9/2023 and concluded on 02/9/2023. The inspector was on site at the center from 11:40am to 1:15pm. There were 62 children in care with 14 staff supervising. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. The center?s playground and all classrooms were inspected today. Six children?s records and six employee records were reviewed during this inspection. Information gathered during the inspection determined areas of noncompliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Based on record review and an interview, the center did not ensure to have at least one person on staff that has been trained to conduct daily simple health screenings of children.
Evidence:
During the inspection of six staff records and an interview with the center representative, it was discovered there were no staff that received training in daily health screenings within the past three years. Religious exempt requirements stipulate that a staff should be able to conduct a daily simple health screening and exclusion of sick children by a person trained to perform such screenings. Daily health screenings should be completed every three years.
Plan of Correction: We will take the online daily health observation training as soon as possible.

Based on record review, the center did not ensure one of six staff records contained documentation of sex offender registry check results and child abuse and neglect search requests from any state in which the individual has resided in the past five years.
Evidence:
1. The record of staff #2 (employment date: 10/8/2021) did not have documentation of out of
state sex offender registry check results and child abuse and neglect search requests.
2. The record of staff #2 indicated that the staff member has lived outside of Virginia within the
last 5 years.
Plan of Correction: We will get the staff member's background check as soon as possible.

Based on observation and an interview, the center did not ensure that the building serving preschool aged children was equipped with at least one carbon monoxide detector.
Evidence:
The licensing inspector did not see a carbon monoxide detector installed in the center. The
center?s administrator stated that a carbon monoxide detector is not currently installed in
the building.
Plan of Correction: We have ordered the detectors and will be installed.
