During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the monitoring inspection. Subsidy standards that are not covered in voluntary registration standards were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 03/24/2025. A POC submitted after this date will not appear on the public website.
Violations
3Vendors shall maintain, and keep at the family day home, written or electronic information for each enrolled child, which shall be made available to the department's representative upon request.
There was no record for Child 2, Child 3, or Child 4 available during the inspection.
There was no immunization record for Child 3 available during the inspection. Child 3 was observed in care.
Caregivers that work directly with children shall annually attend at least 16 hours of training to include the department?s health and safety updated course.
Caregiver 1 and Caregiver 2 did not have documentation of any annual training hours.
