Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 7/24/2024 and completed on 7/24/2024. The on-site inspection began at 9:57 a.m. and ended at 1:25 p.m. The inspector reviewed compliance in the areas listed above. There were 35 children present and six staff. The inspector reviewed six children?s records and six staff records on-site on 7/24/2024. This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/8/2024. A POC submitted after this date will not appear on the public website.

Violations
3Evidence:
1. Broken floor tiles and split flooring was observed throughout the school-age classroom. Tape was used previously to conceal the damaged flooring. The tape was broken, exposing tripping hazards.
2. Damaged vents were observed in the School-age Classroom and the Two-Year-Old Classroom. The vents were bent and lifting from the floor surface.
3. In the Pre-School Classroom there was chipping paint along the windowsills and along the wall.
4.On the outside playground, metal fencing surrounding the perimeters had several areas of untwisting wiring, resulting in an unraveling-spoke; a broken panel on the wooden fence; nails were loose and lifting on the wooden fence.
5. On the outside porch, there was a broken table, accessible to the children outside the Preschool and School-age Classrooms.
Evidence:
1. In the Toddler room, in a cabinet, located in the corner of the room, there were two sanitizing solutions, and an aerosol can room spray.
2. In the Two-Year-Old restroom, in a cabinet above the countertop, there was a bottle of sanitizing solution. The cabinet?s lock was broken.
3. On the outside playground porch, there was an open bag of garden soil. Manufacturer?s warning label stated, ?keep out of reach of children?.

Evidence:
1. The trash can lid in the Infant Room was broken. Staff verified that they had to use their hands to remove the lid with their hands to dispose of the soiled diapers.
2. The trash can lid in the Two-Year-Old Classroom was broken, and on the floor, there were soiled diapers/pull-ups in the trash can.
