Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 11/14/2025 and completed on 11/14/2025, as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the last inspection related to building maintenance. The on-site inspection began at 12:03 pm and ended at 12:17 pm. The inspector reviewed compliance in the areas listed above. The inspector reviewed zero children?s records and zero staff records on-site.
This inspection included a tour of the facility and observations .
