Inspector Notes

An unannounced complaint inspection was initiated on 01/26/24 regarding allegations of non-compliance with supervision. The inspector was on site on 01/26/24 from approximately 10:30 am until 1:30 pm. There were 77 children present, ranging in ages from four months to five years, with 16 staff supervising. The inspector conducted observations and interviews on site. Additional documentation was requested and reviewed remotely. The preponderance of evidence gathered during the investigation supports violation of standards in regard to the complaint. Therefore the complaint is considered to be valid.

Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from today, 02/15/2024. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Based on interviews and documentation review, the center failed to establish and implement procedures to ensure appropriate supervision of all children in care, including daily intake and dismissal procedures to ensure safety of children.
Evidence:
1. Interviews and documentation review revealed that on January 11th, 2024, child #1 age 2 years, ran away from the class and was unable to be located for approximately 5 minutes.
2 Child #1, age 2 years, was found in the parking lot, alone and unattended by another parent who brought the child inside.
3. The center is located on a busy street in the city.
Plan of Correction: GCCDC have begun not allowing teachers to travel alone with students outside of the classroom. Teachers have been and will continue doing head counts before traveling to a different space, another head count wen leaving said place, and another head count when entering back into the classroom. If a child is known to wander during travel, teachers will hold that child's hand and have a meeting with parents about safety of staying with teachers. If a teacher is found not following travel safety procedures, this may result in a write-up or termination of position with GCCDC.
