During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site code compliance inspection was initiated and completed on 10/17/2024. The on-site inspection began at 12:54 pm and ended at 2:00 pm. The inspector reviewed compliance in the areas listed above. The Department requested that the provider complete an IPOC as result of the last inspection related to out of state background checks, background checks, supervision, staff health reports, and handwashing. There were 70 children present with 21 staff. The inspector reviewed 4 staff records on-site. This inspection included documentation review and interviews.
Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/05/2024. A POC submitted after this date will not appear on the public website.
Violations
3Prior to employment each person in a supervisory position shall be certified by a practicing physician or physician assistant to be free from any disability which would prevent them from caring for children under their supervision.
The staff health report for staff #1 was completed 3 weeks after employment.
The center must obtain a completed national criminal background check prior to the employee's first day of employment.
Staff #1, had a completed national criminal background check 1 week after employment.
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.
1. Staff #2, who has been employed for 2 months did not have documentation of requesting a criminal history check or a sex offender registry check from a state that they lived in within the preceding five years.
2. Staff #3, who has been employed for 1 month did not have documentation of requesting a central registry check, or a sex offender registry check from a state that they lived in within the preceding five years.
3. Staff #4, who has been employed for 10 months did not have documentation of requesting a central registry check, or a sex offender registry check from a state that they lived in within the preceding five years.
