During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced monitoring inspection was initiated on 03/07/2025 and completed on 03/14/2025 in response to a self-report received by the licensing office on 03/03/2025 relating to allegations of supervision. The inspector reviewed 1 child?s record electronically on 03/03/2025 and 2 staff records on-site This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 04/07/2025. A POC submitted after this date will not appear on the public website.
Violations
1The center shall establish and implement procedures for appropriate supervision of all children in care.
1. Staff #1 and staff #2 transitioned classroom #1, with 12 children age 2 years, from the classroom located on the "middle" floor via the elevator down to the basement level to go to the fellowship hall.
2. Child #1, age 2 years, was found on the elevator by staff #3 when the elevator returned to the middle floor which is 2 floors above the basement floor. Child #1 was crouched in the corner of the elevator, shaking, screaming and crying.
3. Staff #3 picked up child #1 to comfort the child. Staff #3 took child #1 back to her classroom and reported the incident to administration.
4. Administration reported that when staff #1 and staff #2 were confronted they did not realize that child #1 was missing from the group. Staff #1 and staff #2 stated that they did not conduct the required face to name tracking procedure when they transitioned the class from the elevator to the fellowship hall.
5. Child #1 was out of supervision and alone for approximately 5 minutes.
