Inspector Notes

An unannounced monitoring inspection was initiated on 03/17/25 and completed on 03/18/2025 in response to a self-report received by the licensing office on 03/11/2025 relating to supervision. The inspector reviewed 1 children?s record and 2 staff records on-site and electronically on 03/18/2025. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to staff health reports and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 04/07/2025. A POC submitted after this date will not appear on the public website.

Repeat Violation
Each person in a supervisory position must be certified by a practicing physician or physician assistant on or before the first day of employment to be free from any disability which would prevent him from caring for children under their supervision.

Staff #1, employed for 2 weeks, did not have documentation of certification from a physician or physician assistant to be free from any disability which would prevent them from caring for children under their supervision.
Plan of Correction: Staff #1 was examined by a physician before beginning work. The doctor did not fill out the form. Goring forward we will insist the form be completed.

Repeat Violation
The center shall establish and implement procedures for appropriate supervision of all children in care.

Staff #1 transitioned class #1 from the playground to the classroom. Child #1, age 2 years, was discovered by child #1's parent sitting alone and unsupervised outside of the building. The door to the center was closed. Staff #2 was on the playground with a group of children. Staff #2 was unaware that the child was sitting on the step alone and unsupervised. The parent brought child #1 into the building and to classroom #1. Staff #1 was unaware that child #1 was left outside.
Plan of Correction: All staff have been trained on sight and sound supervision and procedures on transitioning from one area to the next.
