Inspector Notes

An unannounced, on-site renewal inspection was conducted 11/13/2024 from 1:15pm- 2:15pm. The inspector reviewed compliance in the areas listed above. There were 12 children present and 3 caregivers. The inspector reviewed 3 children?s records and 3 caregivers records onsite. This inspection included document review, interviews, observations, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-200-C
The provider shall orient the assistant by the end of their first week of assuming job responsibilities. Documentation of the orientation shall be signed and dated by the provider and assistant.

Occupied cribs, cots, rest mats, and beds shall be at least 12 inches from each other.

During nap time, two occupied cribs were observed to be less than 12 inches from each other.

Cribs, cots, rest mats, and beds when being used for sleeping or napping by children other than infants shall have linens consisting of a top cover and a bottom cover or a one-piece covering that is open on three edges.

During nap time, 4 out of 5 children were observed sleeping without a bottom cover.
