Inspector Notes

An unannounced monitoring inspection was conducted on January 31, 2024 between the hours of 11:00 am through 1:00pm. There were a total of eight children (22 points) observed with three caregivers. A total of five children's files and six staff/household member's files were reviewed. The children were engaged in the following: toy play, lunch, bathroom breaks, hand washing, and other organized activities. Areas of non-compliance are identified in the violation notice. Standard 8VAC20-800-120-B
Based on record review, required records were not kept for each caregiver.
Evidence:
1. Caregiver #2's record (Date of hire-01/16/2024) did not contain documentation of an emergency contact and documentation of two or more references as to character and reputation as well as competency.
2. Caregiver #3's record (Date of hire-01/03/2024) did not contain documentation of an emergency contact and documentation of two or more references as to character and reputation as well as competency.
Plan of Correction: The provider will ensure to obtain each caregiver's emergency contact information. The provider will also ensure to document the records of the caregivers references.
Standard 8VAC20-800-170-A
Based on record review and interview, the provider did not obtain for each caregiver at the time of hire and prior to coming in contact with children, a current Report of Tuberculosis (TB) Screening form.
Evidence:
1. Caregiver #2's record (Date of hire-01/16/2024) did not contain documentation of a current TB screening form.
2. Caregiver #1 confirmed that Caregiver #2 did not obtain a current Tb Screening.
Plan of Correction: The caregiver will get a current Tb screening.
Standard 8VAC20-800-60-B
Based on record review, children's records did not contain required information.
Evidence:
1. Child #3's(Start date-01/09/23), Child #4's (Start date-01/04/23, and Child #5's (Start date-01/04/23) records did not contain documentation of the review of the child's emergency contact information.
Plan of Correction: The provider will have the parents to document the children's records to provide up to date information.
Standard 8VAC20-800-90-C
Based on record review, the family day home did not obtain documentation of additional immunizations for a child.
Evidence:
1.Child #5's record (Start date-01/04/2023) did not contain documentation of additional immunizations. Documentation of the last immunization was dated 11/28/2022.
Plan of Correction: The provider will get the additional immunizations for the child.
