Inspector Notes
An unannounced renewal inspection was conducted on 07/11/2023 from 10:40 am-12:10 pm. There were 5 children supervised by 1 direct caregiver. The home, 1 staff record, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed engaging freely, eating snacks, and playing outside. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. 4Evidence:
1. The most recent criminal history result on file for Caregiver #1 is dated 4/20/2018 and expired on 4/20/2023.
Evidence: The most recent rabies vaccination documentation reviewed during the inspection for the provider's cat indicated that an update was due on 2/9/2022.
Evidence: A stationary slide and climbing playset were placed on a hard surface (dirt) without protective surfacing material underneath the equipment. There was no resilient surfacing present.
Evidence:
1. Child #5's last annual emergency contact review was on 1/24/2022.
