Inspector Notes

An unannounced renewal inspection was conducted on 07/11/2023 from 10:40 am-12:10 pm. There were 5 children supervised by 1 direct caregiver. The home, 1 staff record, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed engaging freely, eating snacks, and playing outside. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Based on record review and interview, background checks were not repeated every five years.

Evidence:
1. The most recent criminal history result on file for Caregiver #1 is dated 4/20/2018 and expired on 4/20/2023.
Plan of Correction: Went and got fingerprints on 7/7/2023 waiting for results.
Standard 8VAC20-800-460-C
Based on review of documentation, the provider did not ensure that pets had current vaccination for rabies.

Evidence: The most recent rabies vaccination documentation reviewed during the inspection for the provider's cat indicated that an update was due on 2/9/2022.
Plan of Correction: I took cat to get shot on 2/14/2023 cant find records will have to give her other shots.
Standard 8VAC20-800-500-C
Based on observation, it was determined that stationary outdoor play equipment was installed over a hard surface.

Evidence: A stationary slide and climbing playset were placed on a hard surface (dirt) without protective surfacing material underneath the equipment. There was no resilient surfacing present.

Based on record review and interview, the provider did not annually review with the parent the emergency contact information to ensure the information is correct and obtain the parent?s signed acknowledgment of the review.

Evidence:
1. Child #5's last annual emergency contact review was on 1/24/2022.
