Inspector Notes
An unannounced monitoring inspection was conducted on 4/19/2023 from 9:22am to 1:00pm. At the time of entrance, 116 children were in care with 21 staff members present. Children were observed playing on the floor with the teachers, listening to the teacher read a book, eating snack, playing outside, washing hands, playing in centers, in group discussion during circle time, singing, reading books on the carpet, drawing and tracing with stencils, playing with table toys, tracing letters, working on an art project with markers, scissors and glue and cutting out objects and gluing them on paper under recycle category or compost category. Infants were observed sleeping, being held and sitting in seats. Walls, bulletin boards and several other areas of the programming space was decorated with artwork, crafts and projects completed by the children. Interactions between the children and staff were positive. The site was organized and contained an abundant supply of developmentally appropriate materials. A selection of staff and children records, medications, the physical space, evacuation drills, emergency supplies and attendance records were reviewed. 5Evidence:
1. Staff #3 (Date of hire 12/11/2022) had a central registry finding dated 2/09/2023. It was not within 30 days of employment.
tuberculosis (TB) test or screening at the time of employment and prior to coming into contact
with children that was completed within the last 30 calendar days of the date of employment.
Evidence:
1.Staff #3 (Date of hire 12/12/2022) had a TB test or screening dated 8/24/2022 and 1/04/2023.
2. Staff #5 (Date of hire 2/27/2023) had a TB test or screening dated 3/02/2023.
operable condition.
Evidence:
1. In the Pandas classroom, a small area of paint was chipping approximately 1/2 inch in size near the floor board under the ABC board.
2. In the Whales classroom, a blue children's chair had a small tear in the fabric approximately 1/2 inch in size.
3. In the Polar Bears classroom, a blue children's chair had a tear/hole on both corners of the chair. One was approximately the size of a quarter and the other was approximately 2 inches in length.
Maintenance has been contacted and will be here by 4/30/2023 to fix the
chipped paint.
We will remind staff at our April 25th staff meeting to report all repairs needed to the management team.
Evidence:
1. In the Chipmunk infant classroom, the changing pad had a small tear approximately 1 cm on the surface. The pad is considered absorbent with the tear.
