During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 5/19/2025 and completed on 5/19/2025, as a part of the licensure period. The on-site inspection began at 10:38am and ended at 1:15pm. The inspector reviewed compliance in the areas listed above. There were 118 children present and 28 staff. The inspector reviewed ten children?s records and ten staff records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
Prior to hire, the center is required to request background checks from any state in which the individual has resided in the preceding five years.

The center requested a search of the child abuse and neglect registry from one state for Staff #10 two days after their date of hire.
Plan of Correction: Will request out of state background checks prior to hire date going forward.
Staff #10 did not inform us that she worked out of state within the past 5 years until
she started.
Standard 8VAC20-780-240-E
REPEAT VIOLATION:
Within 30 days of the first day of employment, staff must complete orientation training in first aid.

Staff #2, who is working with children has not yet completed this course which is overdue by six months and nineteen days.
Plan of Correction: Will provide the required first aid knowledge training and schedule the staff member for First Aid certification. Staff #2 had current CPR certification when she washired; it is still current.
