Inspector Notes
An unannounced complaint investigation was initiated on 6/28/24 with the center?s Directors and owner in response to a complaint received in the licensing office regarding an allegation related to supervision. The inspector was on-site from 10:45am-12:30pm and conducted further interviews off-site. Based on review of documentation and observed evidence, and interviews conducted, the allegation regarding supervision was determined to be valid. Information gathered during the investigation determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program. If you have questions regarding this inspection, please contact the Licensing Inspector, Laura Brindle, at laura.brindle@doe.virginia.gov or 540-905-2062.
Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for the violation cited and return to me by close of business on 7/10/24.
