Inspector Notes
An unannounced inspection was initiated and completed on 8/21/25 in response to a complaint received by the licensing office on 8/12/25 relating to allegations of inadequate supervision, lack of confidentiality, unsafe physical plant, behavioral guidance, notification of child injuries, staff qualifications & training, and ratio. The inspector reviewed one staff record on-site. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
