During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced monitoring inspection was initiated with a local agency on 8/14/25 and completed on 9/12/25 in response to a self-report received by the licensing office on 8/8/25 relating to inappropriate behavior guidance. The inspector reviewed one staff record on-site. This inspection included document review, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and a violation is documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/30/25. A POC submitted after this date will not appear on the public website.

In order to promote the child?s physical, intellectual, emotional, and social well-being and growth, staff are required to interact with children and one another to provide needed help, comfort, and support.

On 8/5/25 during afternoon snack, Staff #1 was observed sitting next to Child A (four-years-old) at the table. Child A put their face close to their bowl of cream cheese and Staff #1 quickly pushed the child?s face into their cream cheese. Child A?s face was observed to be covered in cream cheese. Staff #1 began to laugh and gesture to another staff to show what they had done to the child. Child A reported that they cried and it hurt. Staff #1 continued laughing with the other children even after seeing Child A was upset.
