During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced inspection was initiated and completed on 9/24/25 in response to a complaint received by the licensing office on 9/19/25 relating to allegations regarding ratio and supervision. This inspection included document review, interviews, and observations. The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
