Inspector Notes
A Subsidy Health and Safety inspection was conducted on 1/24/2024 to review supplemental health and safety requirements. There were 45 children present with 9 staff supervising.
A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Based on a review of five staff records and interview, the vendor did not ensure that one staff had current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care within 90 days of employment.
Evidence:
1. The record of staff #1 (date of employment: 8/30/2023) contained an online only CPR certification dated 1/12/2024.
2. The vendor acknowledged that the staff was not certified. Staff #1 was not certified within 90 days of employment and the training taken on 1/12/2024 did not contain the required in-person competency demonstration.
