Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 08/15/2024 as a part of the licensure period. The on-site inspection began at 10:10 am and ended at 12:05 pm. The inspector reviewed compliance in the areas listed above. There were 45 children present and 9 staff. The inspector reviewed 6 children?s records and 6 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/29/2024. A POC submitted after this date will not appear on the public website.

Repeat Violation
Based on record review, and interview, the center failed to obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance.
Evidence: The record for child #3, date of enrollment 01/02/2024, did not contain documentation of a physical examination. Administration confirmed that the record did not contain documentation of a physical examination.
Plan of Correction: The parent was informed that her child needed the complete physical not just the shot record that we had for his file.
Standard 8VAC20-780-500-A
Based on observation, the center failed to ensure that children's hands shall be washed with soap and running water after toileting.
Evidence: A child, age 2 years, did not wash their hands with soap and running water after their diaper was changed. The child was observed getting the diaper changed and going directly to the table to color a picture.
Plan of Correction: The staff were reminded to wash the child's hands along with their own after diaper changes. A new poster was also posted at the changing area as a visual reminder of diaper changing steps and policies.
