Inspector Notes
An unannounced monitoring inspection was conducted today from approximately 10:08 am to 11:20 am. There were 28 children directly supervised by 8 staff. The physical plant, 3 staff records, 2 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. No new medications on-site since the last inspection. Children were observed participating in group play and playing outside. There was an assortment of age appropriate materials for the children in care. The center was clean and organized.

5Evidence:
1. Staff #1 (date of employment 6/20/2023) did not have documentation of a request for a search of the child abuse and neglect registry in record available for review during the inspection.
2. Staff #2 (date of employment 7/11/2023) did not have results of a criminal history record information check and documentation for a request for the child abuse and neglect registry in record available for review during the inspection.
Evidence:
1. Staff #2 (date of employment 7/11/2023) did not have documentation of tuberculosis screening at the time of employment and prior to coming into contact with children.
2. Staff #3 did not have documentation of tuberculosis screening at the time of employment and prior to coming into contact with children.
Evidence:
1. Staff #1 (date of employment 6/20/23) and Staff #2 (date of employment 7/11/2023) did not have documentation of completion of orientation.
Evidence:
1. Only one staff is current with first aid and cardiopulmonary resuscitation (CPR) on premises during the center's hours of operation. All other staff records reviewed expired April 2023.
Evidence:
1. Staff #1 and Staff #2 did not have documentation that two or more references as to character and reputation as well as competency were checked before employment.
