Inspector Notes

An unannounced monitoring inspection was conducted January 2, 2024 from approximately 1:05 pm to 2:05 pm. Upon arrival, Licensing was greeted by Migdalia Blackwell, Director and she is assisted Licensing on the walk-through of the center. There 26 children directly supervised by 9staff. The physical plant, 5 staff records, 4 children's records, evacuation drills, injury reports, emergency supplies, and policies were inspected. No new medications on-site since the last inspection. Children were observed resting. There was an assortment of age appropriate materials for the children in care.

Based on interview and a review of one staff member?s record the center did not ensure that a staff member was not alone with, in control of, or supervising one or more children, without first having completed any required background checks deny continued employment after a Virginia Central Registry finding was not received within 30 days.
Evidence:
Staff member 1 has been employed at the center since June 20, 2023, but does not have documentation of results of a search of the child abuse and neglect from Virginia.

Based on staff records review, the center did not ensure staff complete orientation training in subsection C of this section prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Evidence:
1. Staff #4 (date of employment 6/20/23) and Staff #5 (date of employment 7/11/2023) did not have documentation of completion of orientation.
Plan of Correction: Staff orientation documentation was recorded and added to staff members files.

Based on staff records review, the center did not required documentation fin staff records for each staff person.
Evidence:
1. Staff #4 and Staff #24did not have documentation that two or more references as to character and reputation as well as competency were checked before employment.
