Inspector Notes

An unannounced inspection was initiated and completed on 7/23/2025 in response to a complaint received by the licensing office on 7/23/2025 relating to allegations of handwashing, physical plant, notifications. The inspector reviewed facility records. This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews, observations (this would include observations of photos, recordings, etc.).

The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
