Inspector Notes

A monitoring inspection was initiated on (10/2/2023) and concluded on (10/5/2023). There were 21 children present with 6 staff supervising and 1 administrator. The children were engaged in free play and observed during lunch time and the start of nap time. A total of 5 child records and 4 staff records were reviewed. The inspection started at 9:30 am and concluded at 12 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-260-B
(Repeat Violation) Based on document review and staff interview the provider did not provide annual approval from the health department.
Evidence: The owner confirmed they did not have current documentation of a health inspection.
Plan of Correction: The health inspection was completed on 10/2/2023.
Standard 8VAC20-780-270-A
(Repeat Violation) Based on observation and staff interviews, the provider did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: The Owner verified the following conditions on the outdoor playground:
1. There was a section of black landscape fabric which was protruding from the surface of the ground that posed a trip hazard to children and staff.
2. There was a nail sticking out of a bench seat, under the shade structure, that posed a snag hazard to children and staff.
Plan of Correction: The nail has been taken care of. The fabric will be covered with sand.
Standard 8VAC20-780-80-A
(Repeat Violation) Based on observation and staff interviews the center did not ensure for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
? The attendance sheet in the 2 ? to 3-year-old room showed 7 children checked in when there were 8 children in attendance.
? The attendance sheet in the school age room showed 2 children checked in when there were 4 children in attendance
Plan of Correction: Will make sure it is done from this day forward.
