An unannounced, on-site monitoring inspection was initiated on 8/28/2024 and completed on 8/29/2024. The on-site inspection began at 9:15 am and ended at 11:20 am. The inspector reviewed compliance in the areas listed above. There were 18 children present and 6 staff. The inspector reviewed 5 children's records and 4 staff records on-site. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #3, who had been enrolled for five months.
Plan of Correction: Getting from parent.
Standard 8VAC20-780-500-A
Children?s hands shall be washed with soap and running water after toileting. Staff # 1 changed a child?s diaper in the infant room and did not wash the child's hands.
Plan of Correction: Staff will wash children's hands moving forward.
Standard 8VAC20-780-500-B
Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Staff #1 used their hand to open the plastic-lined storage system to dispose of a diaper.
Plan of Correction: Spoke to staff and it will be corrected.

Staff records shall be kept for each staff person. There was no record available for review for staff #5.
