Inspector Notes

An unannounced monitoring inspection was conducted today from 1:30pm-2:30pm. There were 48 children (infants-5yrs.) directly supervised by 12 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed reading books, practicing tummy time and napping. There was an adequate supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Based on record review and interview, the center did not obtain out-of-state child abuse and neglect findings for an employee that has resided outside the state of Virginia within the last 5 years.

Evidence:
Staff#1 (DOH: 02/14/2022) has resided outside the state of Virginia within the last 5 years, their records did not contain documentation to verify that an out-of-state child abuse and neglect search was completed.
Plan of Correction: We have facilitated the out-of-state Child Abuse and Neglect Findings for our Employee last 3/22/2022 but has yet to receive the result. Upon inspection, we have tried to reach out again to the state's State Police Criminal Information Unit. They have informed us that they will mail it.

Evidence:
The center was not equipped with at least one carbon monoxide detector during the inspection.
Plan of Correction: We have placed an order for the Carbon Monoxide Detector as soon as the inspection was done. It will arrive and will be placed at the facility by tomorrow.
