Inspector Notes

A complaint investigation concerning supervision was initiated on 12/04/2023, from a report received by the Fairfax County Licensing office on 11/28/2023. The investigation was conducted on 12/04/2023, in the morning between 10:57 am and 12:00 pm, and completed on 12/06/2023. The inspection included the review of attendance records, staff records, and interviews with the administration and staff. The preponderance of evidence does support the allegation and the complaint has been deemed valid.

Based on record review and staff interviews, the center failed to ensure that the staff-to-child ratios are required whenever children are in care.

Evidence:
1. On Monday, November 27, 2023, at approximately 4:45 pm, there were 11 infants (ages ranging from 4 months to 1 year old) and two staff in the Infant 2 classroom.

2. During interviews, Staff #1 stated that they left the Infant 2 classroom for approximately two minutes and went to the Infant 1 classroom to get infant bottles and clean up a water spill on the Infant 1 classroom floor.

2. Staff #1 and Witness #1 confirmed that the Infant 2 classroom had 11 infants and 2 staff during the two minutes when Staff #1 was absent from the classroom, exceeding the 1:4 ratio.

3. Staff #1 confirmed that on Monday, November 27, 2023, at approximately 4:45 pm, they left the Infant classroom out of ratio for approximately two minutes.
