During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 12/2/2024, as a part of the licensure period. The on-site inspection began at 10:35am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were 30 children present and 6 staff. The inspector reviewed 4 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The license and any other documents required shall be posted in a conspicuous place on the licensed premises.
The findings from the most recent inspection, dated 5/20/2024, were not posted in a conspicuous place on the premises.
Plan of Correction: Director posted current acknowledgement form from today's inspection and will periodically check board to ensure all required documents are posted.
Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #4 TB screening was dated 49 days prior to date of employment and has been employed for over four months.
Plan of Correction: Director will ensure new staff have TB screening/test completed within 30 days prior to hire and prior to coming in contact with children.

When food is brought from home the food container must be clearly dated.
2:6 bottles with milk brought from home were not dated, in the infant room.
Plan of Correction: The Center will send reminder to parents regarding dating food brought from home.
Standard 8VAC20-780-80-A
REPEAT VIOLATION For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. At approximately 10:30am on 12/2/24, there were 14 children present in the twos room, but only 10 were marked present on the written attendance records.
Plan of Correction: Director will periodically check attendance records throughout the day to ensure accuracy. Director will retrain staff on record protocol.
