Inspector Notes
An unannounced renewal inspection was conducted today from approximately 10:45am-12:10pm. There were 48 children in the direct care and supervision of 12 staff. A sample of 4 children?s records, and 4 staff records were reviewed. The children were observed napping, singing, engaged in an art project, playing on the playground, and playing in the classrooms. One new staff had been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the director. Violations
5Evidence:
Staff A has lived outside of Virginia within the last 5 years and an out of state central registry search was not complete.
Evidence:
1) The closet which kept cleaning supplies was unlocked in the "Preschool 2" classroom.
2) Cleaning supplies were located above the changing table in the "Preschool 1" classroom.
Evidence:
911 and emergency phone numbers were not posted near the telephone in the "Preschool 2" room.
Evidence:
The records of Children A and B were missing their father's employer phone number.
Evidence:
The records of Staff A, B, C,and D were missing a verification of age.
