During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 02/24/2025 , as a part of the licensure period. The on-site inspection began at 10:20a.m.and ended at 12p.m. The inspector reviewed compliance in the areas listed above. There were 48 children present and 10 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Staff #1, employed for more than 8 months, did not have documentation of requesting an out of state child abuse and neglect search.
Plan of Correction: The out of state search will be requested immediately. Results will be sent to the inspector for review.

The center must request a search of the central registry prior to employment and by end of 30th day obtain a copy of the results of a search of the central registry

Results of a Central Registry search was not obtained for Staff #1, who had been employed for 8 months.
Plan of Correction: A search request will be requested again for this staff. Results will be forwarded to the inspector for review.

Equipment available shall be age and stage appropriate for the children.

Manufacturer's age guidelines for the playground equipment is for children ages 2 years to 5 years. A child under the age of 2 years was observed on the playground equipment.
Plan of Correction: Staff will be reminded that only children age 2 and older are permitted on the playground equipment.

Over-thecounter skin products shall not be kept or used beyond the expiration date of the product.

A topical skin product for Child #1 expired in November 2025.
Plan of Correction: This ointment will either be returned to the parent or disposed of.

The center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

There were a total of 12 children present in a classroom. The attendance record for this classroom documented only 11 children in attendance.
