During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 08/06/2025, as a part of the licensure period. The on-site inspection began at 10:30a.m. and ended at 12:30p.m. The inspector reviewed compliance in the areas listed above. There were 49 children present and 9 staff. The inspector reviewed 6 children?s records and 8 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-190-B
Program directors without management experience shall have one college course in a business-related field, 10 hours of management training, or one child care management course that satisfactorily covers the management functions of planning, budgeting, staffing and monitoring. Management experience is defined as at least six months of on-the-job training in an administrative position that requires supervising, orienting, training, and scheduling staff.

Staff #6 is the program director has less than six months of management experience and has not completed a business management course.
Plan of Correction: Documentation of education and the curriculum will be obtained and maintained in the record and also forwarded to the Licensing Administrator for review.

The required ratio for children ages 16 to 24 month is one staff per five children.

Staff #5 was observed alone in the classroom with 10 children between the ages of 16 months to 24 months for approximately 1 minute.
Plan of Correction: All staff in every classroom will be re-trained on the requirement to maintain the staff to child ratio at all times. Staff will alert the director if there is a need to leave the classroom.

The required ratio for 2 year old children is one staff per 8 children.

Staff #7 was left alone with 12 children(2 children 2 years of age), while Staff #8 left the classroom for approximately 30 seconds.
Plan of Correction: All staff in every classroom will be re-trained on the requirement to maintain the staff to child ratio at all times. Staff will alert the director if there is a need to leave the classroom.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization.

Medication for Child #6 was onsite, but the medication authorization had expired two days ago. The medication had not been administered past the expiration of the authorization. The authorization had not been renewed and the medication was not returned or disposed of as required.
Plan of Correction: The parent will be informed that new authorizations are required or the medication will be returned to the parent.

Documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position shall be maintained in the staff record.

Staff #6 is the program director. The record for staff #6 does not include documentation of the required education for the job position.
