Inspector Notes

An unannounced monitoring inspection was conducted today between 11:30am and 1:00pm. There were 12 children present with the Provider and 2 assistant for a total of 30 points within the supervision guidelines. The children were observed playing outside, washing hands, eating lunch (rice soup, cucumbers, green beans, mango and milk). Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Based on record review, each child's record was not were not maintained up-to-date and reviewed annually.
Evidence:
1. Child #6 most recent documented review of the child's record is dated 7/19/2021.
2. Child #10 most recent documented review of the child's record is dated 7/19/2021.
3. Child #7 most recent documented review of the child's record is dated 2/14/2022.
Plan of Correction: I will have the parents review and sign this afternoon.

Based on record review, the family day home did not obtain documentation of additional immunizations for each child under the age of two years.
Evidence:
1. Child #7 (21 months) most recent documented immunizations is dated 8/19/2022.
2. Child #9 (23 months) most recent documented immunizations is dated 5/26/2022.
Plan of Correction: I will text the parents and ask them to bring an updated copy of shots.
