There was no physical exam record available for Child #3 (enrolled over one month) and was overdue by two days.

Assistants' and substitute providers' records shall contain verification of age.

The record for Caregiver #2 (employed over 3 years) was missing verification of age.

Topical skin products such as sunscreen and insect repellent need to be inaccessible to children.

Sunscreen bottles and insect repellent bottles were accessible to children. One bin was located behind a curtain on the bottom shelf of the diaper changing table and two sunscreen bottles were inside an open closet.
