A Subsidy Health and Safety Inspection (SHSI) was conducted on 8/29/2023 from 10:45am-1:45pm. A licensing monitoring inspection was conducted on the same date as the SHSI inspection. The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non compliance with applicable standards and violations were documented on the violation notice issued to the program.

Based on observation and interviews, it was determined that the vendor did not ensure that for each group of children, the vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1. In the 5-7 year old group there was no written hard copy of children?s arrival and departure times.
2. In the 9-12 year old group there was no written hard copy of children?s arrival and departure times.
3. Staff #1 (Program Director) confirmed that there was no written record of children?s arrival and departure.

Based on observation and interviews, it was determined that the center did not ensure that all staff who work directly with children shall have within 90 days of the date of employment, current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.

1. Staff #4, date of hire 9/19/2022 did not have documentation of current certification in cardiopulmonary resuscitation (CPR).
