Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 1/27/25 and completed on 1/31/2025. The on-site inspection began at 9:30am and ended at 11:20am. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 22 children present and 4 staff. The inspector reviewed 5 children?s records; 4 staff records were reviewed electronically on 1/31/25. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/18/25. A POC submitted after this date will not appear on the public website

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. There were two picnic tables on the playground with peeling paint. There was broken plaster on a wall in the classroom.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. There was Raid Max Ant and Roach spray, Tri-Fecta cleanser and glass cleaner in an unlocked cabinet in the classroom.

Cleaning materials (e.g., detergents, sanitizers and polishes) and insecticides/pesticides shall be stored in areas physically separate from each other. In a cabinet in the classroom there were bottles of glass cleaner, cleanser and insecticide stored together.

Staff records shall be kept for each staff person and shall contain all of the required information. The name, address, and telephone number of a person to be notified in an emergency was not kept at the center for Staff #2 and Staff #3. The record for Staff #3 did not contain the date of hire.
