Inspector Notes

Conducted an unannounced monitoring inspection at 10:47a. Observed 26 children + 8 direct-care staff. Ratios and supervision were in compliance. Children were observed playing with toys, making seasonal art, counting, writing the alphabet, having active outdoor play, washing hands, eating lunch and taking naps. Staff were actively engaged with the children and activities. The facility was found to be clean and well supplied toys and equipment for the children. Areas of non-compliance with standards and laws reviewed was found. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Based on observations made, the findings from the most recent inspection were not posted in a conspicuous place on the licensed premises. Evidence: The inspection report dated 2/7/23 was not posted in the building. It was in a notebook kept by the director.
Plan of Correction: Violation #2: Most recent inspection was taken down due to the center being painted and is currently in a conspicuous place in the center.
Standard 22.1-289.035-A
Repeat violation.
Based on records reviewed, 1 of 7 staff records reviewed did not include a criminal record check every 5 years. Evidence: The background check for Staff #3 was last completed 8/16/18.
Plan of Correction: Violation #1: Staff #3 had a current Virginia Central Registry on file at the time of licensing inspection. An updated criminal background check has been completed showing zero findings and shows that staff is eligible to work. Updated Criminal Background check is on file at the center.
Standard 8VAC20-780-130-E
Based on records reviewed, 3 of 4 children's records reviewed did not include documentation of immunizations every 6 months until the age of 2 years old.

Evidence:

1. The immunizations for Child #1 were dated February 2023 when the child was 9 months old.
2. The immunizations for Child #3 were dated January 2023 when the child was 6 months old.
3. The immunizations for Child #4 were dated February 2023 when the child was 6 months old.
Plan of Correction: Violation #3: Child #1 and Child #4 have current immunizations on file at the center and Child #3 has an appointment on 10/27/23 and will supply updated immunizations once the visit is complete.
