During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/24/2025, as a part of the licensure period. The on-site inspection began at 10:55AM and ended at 1:14PM. The inspector reviewed compliance in the areas listed above. There were 36 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/3/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-500-A
Staff shall wash hands with soap and running water before and after a diaper change.

Staff in Classroom #1 and Classroom # 2 did not wash hands after changing a child?s diaper.
Plan of Correction: Staff has been retrained on proper Handwashing procedures.
Standard 8VAC20-780-500-B
The diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use.

Staff in Classroom #2 did not clean and sanitize the changing surface between diaper changes.
Plan of Correction: Staff has been retrained on correct sanitizing procedure after changing diapers.
Standard 8VAC20-780-570-E
Prepared infant formula shall be refrigerated, dated and labeled with the child's name.

There were four bottles unlabeled in Classroom #1's refrigerator.
Plan of Correction: Labeled all bottles during visit and immediately notified parents of proper labeling procedures.
