Inspector Notes
An unannounced complaint investigation with the local agency was initiated on November 17, 2023 from 9:20 a.m. to 11:30 a.m. in response to a concern received by the Valley Licensing Office on November 16, 2023 in the areas of programs including forbidden action and behavioral guidance.
Observation of the program was conducted. Staff interviews were conducted and documentation was reviewed.
Additional interviews were conducted off site.
Based upon the preponderance of the evidence collected during the investigation, the complaint regarding forbidden action was determined to be valid. The complaint regarding behavioral guidance was determined not valid.
Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program. Please provided plan of correction and the date to be corrected by within five business days from today, by January 18, 2024.

Violations
1Evidence:
1. Staff interview revealed that in August of 2023, Child #1 was placed in a high chair for approximately three to five minutes as a means to contain them due to being ?physical with Child #2, jumping onto the blue table, flailing his body?.
2. Interview and documentation confirmed that the child was placed in the high chair outside of the intended purpose.
