Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 11/04/24 and concluded on the same day., as a part of licensure period. The on-site inspection began at 10::50 and ended at 12:35. The inspector reviewed compliance in the areas listed above. There were 3 children present and the providers own child and the provider. The point total was 3. The inspector reviewed 3 children?s records and 1 staff records . This inspection included document review, a tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The three children in care are missing a second emergency contact.
