Inspector Notes
An unannounced monitoring inspection was conducted on 4/19/2024 at 10:20am. There was 1 child (3 points) directly supervised by 2 caregiver(s). The home, 2 caregiver records, 5 child's records, no household member's records, evacuation drills, emergency supplies, and policies were inspected. There were no children's medications on site today. The child was observed playing freely inside the play room and interacting with the caregivers. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Violations
3Evidence: reviewed 5 children's records and found that Child 1, 2, 4, and 5 did not have documentation of the parent's annual review of information. Child 1's last review was dated 1/31/22, Child 2's last review was dated 1/5/2023, Child 4's last review was dated 1/30/22, and Child 5's last review was dated 5/4/2022.
Evidence: identified through the provider's statements that they did not create a menu for the current week/month.
Evidence: the provider did not have a thermometer available to monitor the temperature in the freezer compartment.
