Inspector Notes

An unannounced monitoring inspection was conducted on 4/19/2024 at 10:20am. There was 1 child (3 points) directly supervised by 2 caregiver(s). The home, 2 caregiver records, 5 child's records, no household member's records, evacuation drills, emergency supplies, and policies were inspected. There were no children's medications on site today. The child was observed playing freely inside the play room and interacting with the caregivers. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Based on review, the facility did not ensure that children's records were maintained with the minimum required information.
Evidence: reviewed 5 children's records and found that Child 1, 2, 4, and 5 did not have documentation of the parent's annual review of information. Child 1's last review was dated 1/31/22, Child 2's last review was dated 1/5/2023, Child 4's last review was dated 1/30/22, and Child 5's last review was dated 5/4/2022.
