Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on August 27, 2024, as a part of the licensure period. The on-site inspection began at 9:45am and ended at 11:00am. The inspector reviewed compliance in the areas listed above. There were four children present and three staff. The inspector reviewed five children?s records and five staff records on site on August 27, 2024. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Based on documentation review, each staff member did not submit documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children.
Evidence:
1.Staff #1's record (Date of hire-06/17/2024) contained documentation of a TB screening that was not submitted at the time of employment and not completed within the last 30 calendar days of the date of employment. Documentation of a negative TB screening was dated 07/10/2024.
Plan of Correction: Moving forward, documentation of TB screenings will be submitted at the time of employment and completed within the last 30 calendar days of the date of employment.
