The center is required to obtain background checks from any state in which the individual has resided in the preceding five years prior to employment.
Staff #1, employed for four months, did not have documentation of requesting a criminal history record information check, search of the child abuse and neglect registry, and a sex offender registry check prior to employment. The sex offender search registry check was conducted four months after the first date of employment. Staff #1 was never left alone with children.

Plan of Correction: The criminal history record check, search of the child abuse and neglect registry will be conducted today. Moving forward, the center will ensure to conduct searches of all out of state background checks prior to employment.
