During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on and completed on 4/30/2025. The on-site inspection began at 8:00 am and ended at 9:05 am. The inspector reviewed compliance in the areas listed above. There were 45 children present and 11 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to obtain a completed sworn statement prior to the employee's first day of employment.

Staff #2, who is currently working, did not have a completed sworn statement in their record.

The required ratio for children ages three years old up to school age is one staff per 10 children. The center has a ratio waiver which allows one staff per 11 children.

The three and four year old classroom contained 14 children with one staff member.
