Inspector Notes

A monitoring inspection was conducted on May 24, 2023 from approximately 10:20 AM to 2:00 PM. There were 64 children present, ranging in ages from infant to five years, with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and six staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Standard 8VAC20-780-270-A
Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1. On the playground, multiple nails were protruding from the playground fence. The playground black boarder had bolts that were protruding. One piece of the playground boarder was broken exposing sharp edges.
2. In room #3, the infant high chair seat protector was torn.

Based on observation and staff interview, the licensee did not ensure the center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.

Evidence:
1. In room #2 and room #4, the allergy list was not maintained and kept confidential.
2. Staff acknowledged that the allergy was not kept confidential.
