Inspector Notes
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the department on May 25, 2023. The complaint was regarding behavioral management and forbidden action. The inspector was on site on July 20, 2023, from 2:35 PM to approximately 4:30 PM conducting interviews, completing observations, and reviewing documentation such as staff orientation training and annual training. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid.
