Inspector Notes

An unannounced inspection was initiated on 05/15/2025 and completed on 05/28/2025, in response to a complaint received by the licensing office relating to allegations of forbidden action and procedures for emergencies. The inspector reviewed one child?s record and two staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports portions of the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

The center shall maintain a written record of children?s serious and minor injuries in which the entries are made the day of the occurrence. Staff #1 applied a bandage to the knee of Child #1 on 4/21/2025. An incident report was not provided to the parents.
Plan of Correction: The child did not receive an injury at the school. He was crying and requested a band-aid for an old wound. The teacher applied the band-aid to sooth the child. We will write an accident report, even when the injury did not occur recently,
